Below is a list of the information that you will need in order to file a claim online.
Carrier Name and Phone # - The name and phone number of the Carrier responsible for the transportation of your freight and for the damages incurred.
Carrier's Invoice Number (Pro#) - This is the freight bill number that the Carrier uses to identify your shipment. It is usually found on the BOL, POD and on their invoice to you.
Shipper and Receiver Info - The complete name and address of the shipper and receiver.
Customer Invoice - Your invoice to your customer indicating type and cost of product(s).
Type of Claim - Type of claim incurred; Lost, Damaged, Shortage, Stolen.
*Bill of Lading (BOL) - A transportation document that is the contract of carriage containing the terms and conditions between the Shipper and Carrier.
*Proof of Delivery (also known as Delivery Receipt, POD, DR) - Information supplied by the carrier containing the name of the person who signed for the shipment, the time and date of delivery, and valuable claim information such as indications of Visible Damage or Shortage.
* If you can provide us with the Carrier Name and their Invoice Number (Pro#), we can usually obtain copies of the BOL and POD.
